What Is the User Information Form?
The user information form is one of the most powerful lead generation features in Quizify. It gives you a way to capture contact details from customers while they are engaged and motivated — right before they see their personalised product recommendations.
What the User Information Form Is
The user information form is an optional step that can appear either at the very beginning of your quiz (before the first question) or at the end (after the last question, before the result page). You choose its position in the quiz settings. If you do not want to collect customer information at all, you can disable the form entirely.
The form can collect any combination of the following fields:
First Name
Last Name
Email Address
Phone Number
Because the form appears at the natural moment of peak curiosity — the customer wants to see their results — it tends to convert at a much higher rate than a standalone newsletter sign-up form on your store.
Where It Appears in the Quiz Flow
You can choose where the user information form appears in the quiz journey. There are two positions available:
At the start (before the first question):
Customer opens the quiz
The user information form appears first — the customer enters their details
Customer proceeds through the quiz questions
Customer reaches the result page and sees their product recommendations
At the end (before the result page):
Customer answers your quiz questions
Customer reaches the final question and clicks to continue

You configure the form's position in the User Info Form settings within the quiz editor. The form cannot be inserted between questions — it is either the first or the last step before results.
What Fields You Can Add
You have four fields available to add to the form:
First Name — the customer's given name
Last Name — the customer's family name
Email Address — the primary field used for email marketing and integration syncing
Phone Number — useful if you run SMS marketing campaigns
You can enable any combination of these four fields. You do not need to use all four — in fact, keeping the form short typically results in more completions.
Most stores get the best results by enabling Email Address and First Name only. Adding too many fields — especially phone number — can cause customers to abandon the form before seeing their results.
Why the User Information Form Is Useful
The user information form serves three main purposes:
Building your email list. Every customer who completes the form becomes a new lead. Their details are captured in your Quizify submissions and can be synced automatically to your connected email marketing platform.
Enabling follow-up marketing. Once you have a customer's email address, you can send personalised follow-up campaigns — such as a product recommendation email based on their quiz answers, or a discount code to encourage their first purchase.
Syncing to email platforms. Quizify integrates with platforms like Mailchimp and Klaviyo. When the form is connected through an integration, each new submission is automatically added to your connected audience or list. This means your email list grows passively as customers take your quiz, with no manual import required.
The Form Is Disabled by Default
The user information form is turned off when you first create a quiz. You must enable it in your quiz settings before it will appear in your quiz flow.
To enable the form, open your quiz in the Quizify editor, navigate to the User Info Form settings tab, and toggle the form on. Once enabled, you can choose which fields to display and whether each field is required or optional.
Changes to the user information form take effect immediately on your published quiz once you save them. If you are making significant changes, consider testing in Preview mode first.
User Information Form vs. the Address Question Type
Quizify also has an Address question type, which you can add as a regular question in your quiz. It is important to understand how this differs from the user information form:

Use the user information form when you want to build your email list and capture marketing leads. Use the Address question type when you need a customer's physical address as part of your product recommendation logic — for example, to recommend products available in their region or to pre-fill a shipping address.
